Know Your Numbers

High blood pressure often has no symptoms — that's why monitoring daily matters. Uncontrolled hypertension raises your risk of heart attack, stroke, and kidney damage.

Systolic (top) Diastolic (bottom) What it means
180 or higher 120 or higher Very high. Recheck in 10 minutes then call your care team immediately.
130–179 80–119 High. Keep checking twice daily. If two readings in this range, contact your nurse within 24 hours.
Below 130 Below 80 Great — this is your goal! Keep monitoring daily.
Below 100 Below 60 May be too low. Recheck in 10 minutes, then contact your nurse.

Your home readings are the most important data in this program. Share them by entering into MyChart every day — your nurse reviews them before every visit and acts if something needs to change.

Your Role in This Program

  • Check your BP twice daily at the same times each day.
  • Enter every reading in MyChart — don't skip days.
  • Take your medications as prescribed. If you can't afford them or have side effects, tell your nurse — there are options.
  • Attend your nurse visits every 2–4 weeks. These are short, focused, and billable just like a provider visit.
  • Work on small lifestyle changes — reduce sodium, increase activity, limit alcohol. Your nurse will help you build a realistic plan.
  • Contact your nurse if anything changes — don't wait for a scheduled visit if you're concerned.
🎓 Graduation Goal
BP below 130/80 at your nurse visit and last several home readings in normal range. Your nurse confirms and closes the program — you return to routine provider care.
