Clinician. Innovator. Builder.

Dr. Fardows Salim is a board-certified family medicine and obesity medicine physician who owns PrimeCare Physicians, an independent primary care and weight management practice in Bloomington, Minnesota.

She serves as an Epic Physician Builder and Quality Improvement Lead at M Health Fairview, building clinical decision support tools, SmartSets, Best Practice Advisories, and care workflows natively inside Epic. She has developed standalone SMART on FHIR web applications — HTNCalc, HRT-Calc, and CycleCalc — spanning cardiology and women's health, plus MoodCalc, an interactive pharmacotherapy reference for behavioral health.

She teaches medical residents and students, leads quality improvement initiatives, and is the architect of the HEART Program (Hypertension Excellence through Automated Reliable Titration), a nurse-led hypertension titration initiative benchmarked against Kaiser Permanente Northern California.

"Disability and finances should never limit anyone's potential — that conviction drives everything I build."
