Professional Bio

Dr. Fardows Salim is a board-certified family medicine and obesity medicine physician who owns PrimeCare Physicians, an independent primary care and weight management practice in Bloomington, Minnesota.

She serves as an Epic Physician Builder and Quality Improvement Lead at M Health Fairview, where she builds clinical decision support tools, SmartSets, Best Practice Advisories, and care workflows natively in Epic. She has developed standalone SMART on FHIR web applications including HTNCalc, HRT-Calc, CycleCalc, and MoodCalc, bringing evidence-based clinical decision aids directly to the point of care.

Dr. Salim teaches medical residents and students, leads quality improvement initiatives, and is the architect of the HEART Program (Hypertension Excellence through Automated Reliable Titration), a nurse-led hypertension titration initiative benchmarked against Kaiser Permanente Northern California.
